This service is designed to help those that are released from the hospital transition back into a normal routine. Post-acute care can be obtained at a variety of places such as inpatient rehabilitation facilities (IRFs), skilled-nursing facilities (SNFs) and long-term-care hospitals, as well as home- and community-based services, such as home health and hospice care. Here at Oxnard Family Circle Adult Day Health Care Center we offer post-acute rehabilitation for your loved one. We carry a dual license from the Department of Public Health for both a medical/health and social model. Our medical model offers a variety of post-acute rehabilitation options that may be compatible with your loved ones needs. At our center we have licensed therapist and aides providing services which include both physical and occupational therapy tailored to your loved ones rehabilitative needs. Our therapist’s office is conveniently located near our physical and occupational therapy area, so they can watch, asses and ensure your loved ones progress is on track. Our therapy sessions are offered in groups and one-on-one to cater to your loved ones individualized goals and needs as assessed and determined by their primary care physician. Our nurses station assists with your loved ones post-acute rehab needs as well by offering 7 fully skilled nurses (LVNs & RNs) at our center. Our nurses provide medication management, wound care, take vitals, give injections and work closely with your primary care physician to take orders, implement the orders right away and follow up with their effectiveness. The food served is tasty and dietary. Our Registered Dietitian and Speech Pathologist design individual diets, which are right for your loved one nutritionally, medically and texture wise.
