Oxnard Family Circle is glad to share a great resource in our community: Ms. Kelly Kivlahan a Transition Advocate with Independent Living Resource Center, Inc.
The Independent Living Resource Center (ILRC) is a disability services and advocacy agency that helps people to increase, or maintain, their level of independence. ILRC has a partnership with the Ventura County Area Agency on Aging (VCAAA) called the Aging and Disability Resource Center (ADRC).
The ADRC seeks to break down barriers to community-based living for older adults and people with disabilities and allows for a coordinated point of entry to ensure they have access to the services and supports they need to allow them to remain as independent as possible, thus remaining in their communities.
As the Transition Advocate, Kelly facilitates transitions for individuals from hospital, nursing facility, intermediate care facility, or any other institutional setting back into the home. Keeping people in their communities allows for self-determination, which increases the quality of life thus improving physical health, and is significantly more cost-effective than institutional living. ILRC uses a person-centered approach, with services individualized and based on the patient’s personal goals.
