The position could be offered to a candidate who possesses one of the following: Bachelor or Master Degree in Social Work, Sociology, Counseling, or Psychology

Oxnard Family Circle Adult Day Healthcare Center specializes in daytime care for adults, seniors, and veterans. Social Worker is a member of the multidisciplinary team, which is designing comprehensive Individual Care Plans for the participants to include social work and counseling, skilled care by nursing staff, registered dietitian, physical, occupational, and speech therapists, PTSD and TBI wellness veterans support groups, caregiver support groups.Responsibilities will include case management, documentation, assessments, reassessments, and progress notes. All the documentation and care planning are computer-based.
