Through Enhanced Care Management, we connect seniors to Community Support services to help address their health-related social needs, such as access to healthy foods or safe housing to help with recovery from an illness. Our care managers assist seniors with all aspects of care to help them live in their homes longer.
Our Adult Day Health Care program stresses partnership with the family, the physician and the community, all working to maintain personal independence. Services provided at our center include care that will be adjusted to each individual participant based on their physical, mental, and emotional needs.
At Oxnard Family Circle, our goal is to make life easier for you and your loved one in every way that we can. We provide transportation to and from our center and cover all areas of Ventura County; Ventura, Camarillo, Oxnard, etc. to ensure your loved one receives proper care every day.
